controversial shift: specialist appointments require general practitioner referral first!

Citizens Claim Specialist Consultations Are Exclusively Available Through General Practitioners - Simply a routine check-up with the family physician scheduled.

Plans by the budding coalition between Union and SPD are generating intense debate over the introduction of a general practitioner regulation for insured citizens. A Forsa survey, commissioned by stern at the tail end of last week, sheds light on this. According to the poll, 49% of Germans consider it unreasonable if, in the near future, people wishing to schedule a specialist appointment will be compulsory questioned by a general practitioner, who will then determine if a referral to a specialist is warranted. By comparison, 46% of the German populace support this idea.5% of those surveyed refrained from expressing their opinion. Only gynaecologists and ophthalmologists shall be exempt from this regulation.

In accordance with this proposed reform, anyone insured with statutory health insurance would be designated a general practitioner as their primary care physician, marking the end of the practice of free doctor choice. The blueprint sketching out this change is the work of the health policy working group of the future black-red government coalition. This revamp is intended to alleviate the healthcare system and streamline patient care coordination. Some statutory health insurers, like the Techniker Krankenkasse, have already offered their insured customers a corresponding general practitioner program on a voluntary basis.

abolishing patient freedom? SPD supporters mostly in favor

Advocates of this new, mandatory regulation, such as the chairperson of the doctors' union Marburger Bund, argue that numerous patients currently consult excess doctors - and often the wrong ones. Critics, however, fear that the general practitioner regulation may exacerbate the consequences of the general practitioner shortage, especially in rural areas.

Some notable differences in attitudes become apparent when the survey results are examined based on political leanings. Those in support of the new general practitioner regulation mainly include SPD voters (57%) and Union voters (52%). The plans are largely rejected by voters of the Greens (51% opposed, 41% in favor), the Left (60% opposed, 33% in favor), and the AfD (56% opposed, 41% in favor).

Healthcare Insights:

  • Specialist Gatekeeping: The proposed policy aims to regulate specialist access through primary care physicians (general practitioners), following a model where GPs act as gatekeepers for specialists.
  • Enhanced Primary Care: If implemented, this policy would result in a significant shift in focus towards primary care, potentially easing the burden on the specialized healthcare system.
  • Potential Wait Times: By requiring referrals to specialists, the policy could inadvertently result in longer wait times for specific medical treatments.
  • Cost Efficiency: Advocates claim this change may lead to a reduction in healthcare costs due to cuts in the use of expensive specialist treatments. However, there could be delays in providing adequate care, which may have long-term financial implications.
  • Pharmaceutical Usage: Reducing specialist consultations may lead to decreased prescription volumes for certain medications, especially novel and costly therapies. This could impact the drug market.
  • Shift to Outpatient Care: Encouraging outpatient care may help optimize resource use, especially as outpatient procedures gain traction with the expansion of Hybrid-DRG reimbursement systems. This transition could serve to improve the overall functionality of healthcare services. It is vital to strike a balance between cost efficiency and maintaining high-quality patient care in implementing this new system.
  1. The Commission, in light of the ongoing healthcare reform debate, has also been consulted on the draft budget regarding the mandatory requirement for specialist appointments, which requires a general practitioner referral first.
  2. Notably, the opinions of specialists might play a crucial role in dividing public opinion on this controversial shift, as the proposed regulation could potentially impact the efficiency and division of care within the healthcare system.
  3. Upon the implementation of this mandatory regulation, it would be mandatory for visiting specialists to obtain an opinion from a general practitioner prior to the appointment, thereby aligning with the concept of primary care as a gateway to specialized care.
