A damning report released by the US Senate has shed light on a series of preventable security lapses that occurred during the preparation and execution of a campaign event in Butler, Pennsylvania, where then-presidential candidate Donald Trump was shot. The incident, which resulted in one death and two injuries, nearly cost President Trump his life.

The report, compiled by the Senate Homeland Security and Governmental Affairs Committee, describes the incident not as a single mistake but a cascade of failures marked by a complete breakdown of security at every level. Key causal factors include bureaucratic indifference, lack of clear protocols, communications breakdowns, refusal to act on direct threats, pattern of negligence, and inadequate consequence management within the Secret Service.

The report emphasises that these failures were "inexcusable" and highlights continued difficulties due to reluctance from agencies such as the FBI and DOJ to share vital information, hampering progress toward fully understanding and rectifying the security lapses.

One of the most concerning findings is the refusal to act on direct threats. Despite having information pointing to potential threats, those in charge failed to take appropriate preventive measures. This pattern of negligence extended to the Secret Service's discipline and enforcement of security standards.

The report also faults the Secret Service for failing to fire personnel after the incident, indicating a lack of accountability. Kimberly Cheatle, the then-head of the Secret Service, resigned shortly after the shots were fired at Trump.

The incident began with the suspicious person involved in the attack being reported to the Secret Service at least 25 minutes before the incident. Despite this, there were serious flaws in the coordination and communication between the Secret Service and state and local law enforcement agencies.

Trump was injured in the right ear during the attack. Disciplinary action was taken against only six Secret Service employees following the attack.

The Secret Service is responsible for the protection of the president and high-ranking politicians in the US. The report serves as a stark reminder of the importance of effective security measures and accountability in protecting our political leaders.
