Transition plate at entrance of walk-in cooler is loose and traps water and debris, posing a potential insect breeding area. Containers on drying shelf were observed to have food residue and were not clean to sight and touch; removed for cleaning.

Soap and towels were not available at the hand-wash sink in the food prep area.
