Canyon Home Care expands across Northern Colorado with key acquisitions
Canyon Home Care & Hospice has expanded its services across Northern Colorado after acquiring Columbine Poudre Home Care and Bloom at Home. The move aims to improve care availability and ensure smoother support for patients in the region. The acquisition brings together two established providers, increasing Canyon Home Care & Hospice's reach to 12 locations. Services now extend to Fort Collins, Greeley, Loveland, and the wider Northern Colorado area. Patients currently under Columbine Poudre Home Care or Bloom at Home will experience no disruption during the transition.
The change is primarily administrative but will enhance operations and resource support. In 2025 alone, Columbine Home Care delivered over 10,000 visits, while Canyon Home Care & Hospice provided more than 49,000. The combined provider now offers medical and non-medical home care, hospice, infusion services, and medical equipment support. Holly Johnson, formerly of Columbine Home Care, has joined Canyon Home Care & Hospice as part of the transition. The acquisition reflects rising demand for in-home care across Northern Colorado, from Wellington to Longmont.
The expanded network will strengthen care continuity for patients and families in the region. With broader coverage and additional resources, Canyon Home Care & Hospice now serves more communities across Northern Colorado.
